I need to get some blank blue light permits for the fire department. When I joined the dept they already had some and since have ran out. No one can recall where they got them so hopefully someone here may have an idea?

I tried the local DMV and DOT and they had no idea so not sure where to go from there.

Ours came from the DMV. I will try to get you a form number off of ours to request

The form is 411022.
"Certificate for authorized emergendy vehicle for blue light"

They are only issued to the fire chief of you dept from Iowa DOT.

here to help as always.

I'd really like to figure out how some in western Iowa are running red/blue's though. I guess it's a loophole in the law and only for officers of the dept as I know one fire chief using both in his POV

Which DOT office will the Chief need to contact? I have heard Ames and another said Des Moines. He's busy enough as it is he doesnt need to be sitting around calling different DOT offices all day.
