My department is looking at a fitness test for all personnell on an annual basis looking for what other departments do.

My department does one each year. It's the same one we did to hire on. You do: hearing test, vision test, color blindness test, lung capacity test, stress test, crunches in 1min, pushups in 1min, sit-and-reach test, body composition with pincers, grip strength, blood/urine workup, and meet with a doctor. I think that's all of it.
There's a point system for flexibility, p/u, crunch, stress test, and body comp. Based on those points you're ranked superior, excellent, good, fair, poor. If you're fair or poor they notify your batt chief and you have to go back and redo it in so many months. If you don't make "good" after that you go to the training center where they put you on a PT program.
The test is out-sourced to a local hospital that runs an assessment lab.
