I apologize if this topic has been beaten to death, but I think my situation is a bit different than most that I have read

I'm a paid on call firefighter for a busier combination career/paid on call dept. I've worked there for over 4 years. I'm currently in line to get offered a full time spot in the next month or two. The dept currently only hires firefighters that are already paid on call with the dept.

On to my question... I will have to take a psych/pre employment physical exam before officially being hired. Problem is, I have had a couple of medical problems over the past couple of years, one of which kept me off the job for about a month. (should be noted that I'm back to working my regular shifts as a paid on call, no restrictions)

Here's the brief description of what happened: In early 2007, I experienced some neurologic type symptoms that could have been rather serious, possibly related to scary-type diseases. I saw a neurologist. The doc kept me off of work until the very scary stuff was ruled out. I had several tests done, mri of my head etc. Nothing was found, symptoms abated, I was cleared back to work full duty, no restrictions. Also during this time, I started with lower back pain. I must admit I probably over reacted and went to a back specialist. MRI was done, found a bulging disc L5-S1. Doc found no reason for surgery, no reason for any procedure other than pt to strengthen my core. Did that. Was never taken out of work due to back issue. I've been working ever since, sometimes the back gets sore, but nothing to write home about. Also, in the last year, I finally got a diagnosis on my neurologic from a very highly regarded specialist. Bottom line, it's a very mild problem, that will most likely never get worse, and may very well likely go away on it's own.

So, how honest should I be at the physical exam?? It should also be noted that the 2 specialists I saw both agreed to provide me with letters stating I would be fine to pursue my career interest as a firefighter. I've had varying opinions on this. I'm just very worried because there is quite a bit of medical history attached to my name, I do not want to get myself in any trouble. Just wondering if I should worry less because I'm already on the dept for the past 4 years and they are already somewhat aware of what was going on...thanks for any input...
