Background Investigation questionaire

My PM inbox has been filling up with questions regarding babckground investigations. How much do you reveal, what do I have to tell the investigator, and will they find out?

I am going to repost the Background Investigation questionaire so you can get a feeling of what the investigator will ask.

Keep in mind a couple of things:
1. Nobody is perfect
2. Intentionally with holding or omiting information is the same as telling a lie.
3. Lying is grounds for immediate dismissal from the background process.
4. There are some issues that will keep people from becoming a firefighter.
5. There is no such thing as an expunged record.

This is not my department's form, nor did I make this up. This form is was given to me by the Fire Chief of a major California Fire Department who frequents this board. He asked me to remove his department name and circulate this to aspiring firefighters.

my form a far more involved than the one posted, it asks me for all my roommates info I've ever had, which I just can't remember.

I understand that omitting anything seems like a lie, but for me I live in a house ten years ago with four others that i haven't spoken to since, I'm happy to mention that but I honestly have zero memory of their phone numbers, and some of their names, I have nothing to hide, but genuinly cannot recall the info, how can I handle this?
