Part of the bucket got caught on a parapet wall. The operator tried to free it, resulting in the device bending and storing a massive amount of energy.
When the wall/bucket released from each other, the ladder whipped violently, and two FF's struck the platform door, bending/breaking the block that prevents the door from opening outwards.
When the door opened outwards, it allowed and/or contributed to the two FF's falling out.Last edited by txgp17; 07-18-2009 at 03:51 PM.

With all due respect to our fallen brothers, there is a critical part that you fail to note. Neither of those who fell were wearing any type of ladder belt. That is Basic when operating on ANY aerial. While there may be some fault with the apparatus, the report focuses on personnel error as the main cause. Belts would have saved them !
