Investigation checklist

I am looking for a checklist/question form that each investigator would have to help them do a proper investigation. Our dept has a few guys trained as investigators and they are part of a county wide team. But we don't have a uniform report for each dept. Something that covers the basics ie: power on/off, anyone home, time of day etc. This is so every dept does it the same way when they check out a fire. This is the first placed I looked, so if nfpa or someone else has something let me know.
