help me fill out 1199a plz

so i got my 1199a yesterday. under name of person(s) entitled to payment they have my name listed (I wrote the grant and am the primary contact). however i have no power within the dept when it comes to money. our president can sign checks and the treasurer has full power over our bank accounts. who's info should i use and should i have them sign as joints?

Since the treasurer has full power on the accounts, he/she would be listed as the one's entitled to recieve the payment. If the president can make deposits as well, in the absence of the treasurer, have both sign the form.

It's pretty easy to just take it to your bank and let them fill it out. They'll have all the information you need and should be happy to help you out.
