Fire safety/prevention survey for students

Afternoon all,

Our department will be visiting our local high school later this spring to provide a fire prevention program. One thing we would like to do prior to our program is evaluate the current fire safety knowledge of the school students with the use of a survey. I've searched through Firehouse and the internet and have only seen references to surveys, but have not actually seen a survey or the questions asked on a survey.

Just wondering if anyone here can help me out and/or point me in the right direction.
