It seems that in every other NIOSH LODD report one recommendation addresses a "lack of Team continuity". This usually follows a death when a ff becomes separated from his fellow ff's.
Team continuity is not a new process but has continued to be a significant factor in many instances.
Can folks describe training they utilize to enhance and support the concept of Team continuity? Something besides basic 2 in-2 out evolutions.
