Pre-Incident Surveys

My department is in the process of starting to gathering Pre-Incident Surveys for the commercial structures in our first due areas. I am suppose to be heading up the program, i was wondering where i could start, Where to obtain forms and other paper work for it. Also After all the info is gather i believe that it is going to be entered in to a toughbook on our truck, is there a certain program that your department uses or prefers?
