Looking to upgrade our proof of training form for monthly training. Currently just a title and a sign-in sheet. Would like to integrate training agenda and goals, wondering if anyone has anything they would like to share before i go and re invent something.

I know this topic has been sitting for quite some time, but here are some things off the top of my head:
Instructor credentials
Objectives
Course outline (if using PowerPoint, easily printable)
copy of written test questions or copy of skill check sheet
course sign in sheet with student test/skill results

While you're looking at this, have a look at NFPA 1401. It has some templates in the appendices and it's what ISO wants to see in your training records. If you're going to record it, may as well do it so ISO will give you credit.
