We based ours on a mix of questions from recruit prep books. Pick up a copy of Barrons or the many other books like it . They have great sample tests that you can use to put together something of your own.
Why would you test for membership in a Volunteer FD? You're going train them anyway.

We don"t "vote" members into our department. Our process is much like a career FD. We advertise, accept applications, conduct a basic written and physical test, then interviews... Candidates are selected based on their overall performance, and some variables such as daytime availibility.
Probie period lasts a year, and they can be dismissed at any time.
