Hey All,
Keeping in line with a previous post, I was able to find some documentation on aerial failures. This was published in 1996 by the U.S. Fire Administration. Although the documented failures occurred in ladders that by now are 40 plus years old, there is still some very good information on aerial physics. Of note is one incident in Mundelein, Il caused by rope rescue operations (training). See link below (previous link no good, try this one)
