Our dept. has been considering a survey of those who have had the need to use the dept sevices,( both fire and ems). Is anyone out there doing this? What are the questions you are asking? What has been the public response? Thanks for your input.

We do that here in Okdale. I'll make a copy of the card and e-mail it to you. It is printed on a 3x5 card with postage. We get a pretty good response. We also provide a medical info card that provides for their name, who to contact in case of emergancy, doctor, and medical hx with meds. Works great for "frequent flyers", and elderly, especially if they are disabled.
