    Lightbulb Dive Sheet Forms

    Looking for some ideas on preparing a dive log form we can use on our dive team incidents. Do other dept.'s that have dive teams use a type of form that would be used for command info? We are thinking of having vital info concerning the scene, small graph area to draw up the scene, log bottle psi's, ect. I would like to see other forms to compare and get ideas.     Dive Rescue International's DR1 class comes with several good sheets. You can order the student pack from their website.
