Common Mistakes on the 1199A form

Box B "Persons Entitled to Payment"

Is someone who is on the signature card at the bank for that account. Or someone who is entitled to make withdraws and deposits int he account.

You must use the "Print SF1199a" Button to print the form. That button will be on you screen after you submitt the form by clicking "Save & Continue" and on the following page clicking submit.

Forms that are mailed in to FEMA with whiteout or something scribbled out on the form will not be accepted.
