performance evaluations

Greetings,
I am trying to find an evaluation form to hand out to the poeple under me to evaluate the job that I am doing. I have not been able to find one that will meet my needs. Can anyone help?

I created a rating scale for different qualities that subordinates can use to "evaluate" their instructor, also a form for peer evaluations (i.e. Instructors evaluating Instructors, Cadets about Cadets, etc). If you like, I can send them to you.

I would really appreciate you e-mailing what you have. I have also gotten ahold of several other departments locally and they have sent copies of their forms.
