I am thinking about producing a form that I can fax to the newspaper, tv stations in my area in reference to calls that my department goes on.
What is your thoughts. Has been favorable to your department. Can you email me an example of your forms?

Get me a fax # and I'll fax you the one that I was given by Philadelphia's PR department. I can't e-mail it since I don't have it on disc.

As a former newspaper writer, now freelancer, I can tell you that newspapers will love a regularly-submitted fax regarding your department's calls. Small hometown newspapers especially will appreciate your efforts.
