In need of assistance

I'm trying to find or get ideas for a firefighter skill sheet that would be given to my firefighters listing the requirements for training over a three year period.

Does anyone have something like this that they wouldn't mind sharing?

I'm in the process of reviewing the Job Performance Requirements for Colorado, and put something together, but having difficulties finishing it. This needs to be a sheet that they would get signed off upon completion of a task, and it would then be turned into me for records retention.

Would you please e-mail me a copy of what you have so far. Once I see it I may be able to help. Trying to figure out a training spread sheet for our department so we can track who has/needs what. Any suggestions?
