Try www.amtechusa.net for one. EVI and most of the major manufacturers also build what you are looking for.

If your still interested try to call Krammes Kustom Body Company. They do alot of these type of vehicles. They recently did a CSI truck for Clarkstown,NY. Mid Range Rescues
These types of rescues, with the availability of some GVW from Ford on the F-550, have seemingly gained some popularity. Pretty nice rigs for the average department who wants to carry a few people and have some nice compt space. The crew module has a bench seat for three in SCBA's and an area to hold patients to prep for transport via ambulance. The crew area has heat and air.
Thought I would share one....I have many more of these also, but there are "several" builders out there (and I mean legitinate builders) that offer these.
