What this article does not mention is that a Richmond FF also died in this incident. He was on the first due and was lead off the truck.
WCB recommends $130,000 fine

RICHMOND -- The Workers' Compensation Board is recommending a New Westminster company be fined $130,000 in the deaths of five men who were working on a barge two years ago.
The men suffocated inside the barge while conducting maintenance work in New Westminster.
An investigation by the WCB found that Westminster Marine Services lacked a system for identifying and assessing hazards in confined spaces and eliminating or reducing those hazards.
It says the company also did not train all its workers properly.
The WCB is seeking the $130,000 fine and has ordered the company to develop a better policy to deal with working in confined spaces.
Crown officials decided earlier there would be no point in laying criminal charges against Westminster Marine.
