Turn-Out Gear Testing Survey

My department will be testing turnout gear in the near future. Since there seems to be alot of forum members with experience in this area I was wondering if I could ask for some assistance. We have the testing procedure almost finalized but we dont have a survey to ask the testers opinions, likes and dislikes.
