1199A - "Person(s) Entitled to Payment" ??

On the update 1199A page, one item is for the "Name of the person(s) entitled to payment", with fields for a first/MI/last name. Does anyone know if this is a name that must somehow match the banking records or if it simply needs to be a designated officer representing the department? Our department's funds are held in an account controlled by the municipality, the account is not in the department's name. On our 1199A the "person" described above is currently the Fire Chief, but since his name probably is not attached to the bank account, I am stuck and unsure if I can submit the updated 1199A. I got a message from the AFG indicating I needed to submit the updated 1199A, and I want to get it done ASAP but also get it done RIGHT.

I called the AFG help desk and was referred to an "1199A specialist", but got voice mail. That was many hours ago and no callback yet. I guess I am just getting jumpy and impatient, has anyone else had long delays getting a callback from there?

Had the same issue last year. Use the person in the municipality who is in charge of the money. In our case it was the city clerk.

We did the same thing last year, that person or persons have to have their name on the account or be able to access funds in the account listed. They do check it, we listed out secratary and fire chief last year and had to re-do it.
