    Lightbulb Run Sheet / Incident Information

    I'm in the process of creating a two page form that will contain pertinent information about most incidents such as PI's or MVA's, Vehicle Fire, structure fires, etc.

    This form will be used by the engine/truck officer to gather information that will be used to complete the NFIRS forms back at the station.

    I'm looking for suggestion or examples of what everyone else is using for this purpose.

    NFPA 906 "Fire Incident Field Notes". Has forms for different types of fires as well as death/injury and insurance forms. Very handy.

    i utilized the "email" button at the bottom to send you an email. I will send you our old form (Texas version). We pretty much quit using it for some reason. Most of us just write on the back of one of the other sheets in the binder.

    If you don't get the email, post here and we can work on it.
