We have started a new Hazmat team in my county and we are in need of research materials and forms for Hazmat calls. Can anyone help me with this information?

The IAFF books have some good forms in them. They are in the Incident Management section. We use these on our truck, with some slight modifications. I'll try and find some that I can email to you.

Research Materials
Operation Respond @ oreis.org has a great computer software program. It includes the CHRIS manual and Association of Amercian Railroads hazmat book.
You can order NIOSH handbooks as well as the handbook software from http://www.cdc.gov/niosh/npg/npg.html
Hazard materials software may be ordered from the USFA @http://www.usfa.fema.gov/applications/publications/display.cfm?sc=16&mc=33&ol=.
There are a host of hazardous materials reference material out there but those three items listed above along with the ERG 2004 provide enough information for most hazmat responses.

There also used to be a printed version which I can't seem to find at the moment.

To suppliment your books, I would also recommend the following free computer programs that you can download:
CAMEO
MARPLOT
ALOHA
Chemical Reactivity Worksheet
WISER
You can also request a CD from NIOSH containing the latest NPG.
I would also suggest buying the Emergency Response Guides from the Bureau of Explosives and the CHRIS Manual from the U.S. Coast Guard.
Even thought I find computers very helpful, I always like to have hard copies of reference materials, too. Murphy's Law you know.

The Cameo/Marplot/Oloha software is free. The three day class sponsored by the EPA is free of a registration fee. Trainex.org, a EPA contractor also has free training available. Look at their website.
