    Default Incident Reports

    I am currently in the process of creating a new incident report sheet for our department. The sheet we use now is standard form that we began using when we started reporting through NFIRS. The only problem that we have with it is most of the guys leave out key info, mark a field wrong, leave out a module. I wanted to make a more generalized form, where they could just "write-in" most of the info instead of checking boxes. Thanks!

    Our report is pretty simple the front page consist of place, type of call,action taken and patient info. this is set up to checkm off the items. The back has a roster to record who was at the call then a narrative section to explain the call.

    E-mail me or something and I can send you a copy of what we use. I had a similar situation and made one up you can feel free to cheat off of and use whatever you like from.

    We have been using FIREHOUSE Software to do our reports. We do all run reports on the computer in the firehouse. It is the same forms that are required by NFIRS. We got away from having paper incident forms. It is actually easier to do on the computer rather than by hand.
