I'm looking for contact information on any Indiana volunteer fire dept that has acheived 501c status with the IRS and also recieved an employee Identification number. Forms 1023 and the SS-4 are the forms that I am required to fill out. The 1023 is a long form looks like 20 plus pages, our volunteer fire department has been operating since 1837, however we have never pursued 501c status. If someone out there has been through this process and could get me a copy of their application to use as a guideline in filling out mine or could get give me some advise I would greatly appreciate it.
