Default Line of Duty Death SOG

How many departments have a LODD information packet for each individual to fill out in the case of their own death??

This form would take care of the hard questions for the family of funeral arrangements, who should be notified, how they should be notified and who should accompany them for the notification, type of funeral (FD honors or not) and any other special wishes.

When I was on the Sheriff's Dept here we filled one of these out and it was sealed, and placed in our personal file and only pulled out in the case of the event.

I'm sure most career departments have this on file. Just curious how it is set up and what is asked to make our own. I know it's not the best thing to be talking about, even morbid really. However, I would rather be prepared and make it as easy on the family as possible.

    thanks for the help!

I most aggree, I rather know my family will have an idea what to take care of and how to do it in a time of hardship.

Contact the National Fallen Firefighter's Memorial Foundation, they have what you are looking for. They have packets and guidebooks for all of this available.

I should have mentioned this but I did that. I got some really good information but I was hoping for some examples of what departments had in place already.

They got good info, but I wanted just a bit more. Thanks man!

NFFF guidelines, plus we're small enough that the notification of kin would be done appropriately. As it was last fall .
