I am looking for a sample equipment agreement for volunteers to sign when we issue gear, radios, keys, etc. There has been times in the past where members quit the department and/or left town with all the gear and radios. At least if they signed an agreement listing the equipment with serial numbers we would have a little more leverage to get the police involved to track them down. It gets very expensive to replace gear, radios and locks! Any thoughts?

A nice letter asking for return stating if not back by xxxx a uniform will be informing them of court date.
It has work in the past.

Every member signs a loan card when they join. It states every peice of equipment that we issue them. When the time comes that they no longer are on the dept they turn it in. If they don't it starts with letters, followed by an Officer going to the home and lastly if they refuse to turn in the stuff the police. The guys in blue generally get it back.If someone with multiple personalities threatens to kill himself, is it considered a hostage situation?

Here is ours...
I understand that this equipment is merely loaned to me for use in my duties and remains the property of the Borough of Point Pleasant Beach and will be returned by me to Ocean Fire Company #1 at the direction of either the Fire Chief, the Board of Fire Officers, Ocean Fire Company #1's Captain, or the Mayor and Council of the Borough of Point Pleasant Beach, upon written notice to me at the address given below. This equipment shall remain in my custody and shall be used only for official Fire Department business. Further, upon termination of my activities as a Volunteer Firefighter, this equipment will be returned by me to the Captain of this Fire Company within one week of said termination.
We then list the items and serial numbers on the form. It gets signed, copies filed, etc.
And yes, if the person changes address and/or replaces equipment, a new form is filled out."This thread is being closed as it is off-topic and not related to the fire industry." - Isn't that what the Off Duty forum was for?
