Turnout Gear Inspection Forms
Try calling Shamrock Gear at 1-330-313-1220 and I am sure he will povide you with some forms. I took their class on NFPA 1851, gear cleaning and inspection and one of the things he provided was their inspection forms. If you would like to email him, go to http://www.shamrockgear.org and his email address is on the site.

We professionally clean, repair, alter and inspect turnouts. We've done it for 13 years and work closely with the major manufacturers. We have a video on how to inspect your turnouts at home or at the department to determine if you need to have them repaired. This is not a substitute for an advanced inspection. Those should be performed by a third party such as ourselves to divert liability away from you or the department. See those videos here:
http://www.youtube.com/watch?v=yIcEzLXLSXc for the shells and here:
http://www.youtube.com/watch?v=SstxaUYIjmMfor the liners (very important)
Many times the liner will wear much faster than the shell so check those carefully. Check out our website at http://www.freds2therescue.com . We offer free quotes before work will begin if you wish and 72 hour turnaround on washing and inspections. Repairs and alterations, around 5 working days. Call me toll free if you have questions. 1-888-606-7446 Freds 2 the Rescue
