Hi all,
I'm looking to get the CT FF plates for my car, but the CT DMV website only tells me to "contact your local fire chief." My chief hasn't a clue as to what form is needed for it, and I've spoken to FF's who have the plates, and they say there is a specific form needed.
Anyone know where I can get this form?

The links brought me to the same pages I was able to find on my own... the form I was looking for was the one the chief needs to fill out, which isn't on their website.
They don't have the forms at the local DMV, but I was able to call the "Special plate dept" and they sent a pack of the forms to my fire house... took about 4 days to arrive.
