have to disagree with you chief; S is not signs and symptoms (all your textbooks are WRONG) Symptoms, yes; signs - not if it is history!!!

    OPQRST should read
Onset as a "what" question; not a "when" question
    Provokes and Palliates
    Quality
    Region and Radiation
    Severity
    0-10 is worthless
    "None, Mild, moderate, severe, extreme"
    Time:
    Began
    Changed, better or worse
    Ended

    Medications
    Street
    Herbal
    Over-the-counter
    Prescription

Have to disagree with both of you. He did not ask for anything related to EMS at all anyhow.

    Balivation
    Lacrimation
    Urination
    Defecation
    GI issues
    Emeseis
    Eiosis

Thing to looks for in a hazmat chemical exposure situation.
