ABC:

SAMPLE - Signs & Symptoms, Allergies, Medications, Past Medical History, Last Oral Intake, Events leading up to the Emergency.
OPQRST - Onset, Provaction, Quality, Raditation, Severity, Time.
DCAP-BTLS: Defomities, Contusions, Abrasions, Punctures, Burns, Tenderness, Lacerations, Swelling.
Causes of coma - AEIOU-TIPS:
Acidosis / Alcohol
Epilepsy
Infection
Overdose
Uremia
Trauma
Insulin: too little or or too much
Pyschosis
Stroke

have to disagree with you chief; S is not signs and symptoms (all your textbooks are WRONG) Symptoms, yes; signs - not if it is history!!!
OPQRST should read
Onset as a "what" question; not a "when" question
Provokes and Palliates
Quality
Region and Radiation
Severity
0-10 is worthless
"None, Mild, moderate, severe, extreme"
Time:
Began
Changed, better or worse
Ended
Medications
Street
Herbal
Over-the-counter
Prescription
