WEST DEPTFORD TWP. -- Fire officials are responding to the state's scathing investigative report against Verga Fire Co. in an effort to address concerns following the 2001 death of a veteran firefighter.

Released in December, the New Jersey Division of Fire Safety report identified three contributing factors in the March 2001 death of James Heenan. A failure to use the proper equipment; the incident commander's improper analysis of fire conditions; and a steady stream of water, which pushed the fire and super-heated gases on Heenan, were said to all contribute to his death.

The report also said only five of the department's 22 members were certified with Firefighter I.
