On March 30, 2006, the Bismarck Fire Department in Illinois was dispatched to a motor vehicle collision at State Route 1 and Bismarck Road.

Upon arrival a 4 door GrandAm was found under a semi trailer that was hauling 180 hogs. The GrandAm entered under the semi's passenger side and took the second impact in the driver's door from the rear tires and was then drug across the intersection.

Using two sets of Holmatro extrication equipment extrication was complete in 19 minutes and the patient was passed straight into the hands of Medix ambulance the Paramedic service out of Danville Illinois.

In order to gain access to the patient the entire passenger side was opened and the back driver door was taking off and the driver was then passed out the driver side due to lack of space between the semi trailer and the center console of the vehicle.

The patient suffered multiple injuries and was transferred to a Level 1 Trauma center for immediate surgery.

Making this a difficult extrication was the trials of biohazard coming from the trailer due to the hogs and being at a very busy intersection.
