In the late morning of April 22, 2005, Tallahassee Fire Department Engine 9 and 2 received a call to Interstate 10 involving a van and a semi.

According to accounts, a large passenger van was traveling east on I-10 and was cut off by another vehicle. The driver over corrected, causing the van to enter the median and roll several times, ending up in the West bound lane. At this point, a semi truck smashed into it causing the van to explode, ejecting one passenger and causing the semi to push the van back through the median across the eastbound lane and bursting into flames.

Upon Engine 9 arrival, Lt Brady called for a tanker and a third Engine. After finally extinguishing the fire, one fatality was found in the road, and a second victim was in the van.

Due to damage it was unclear if any others were inside.

The driver of the semi escaped with minor injuries.

I-10 was diverted for several hours while the investigation continued.
