On June 22, 2004 at 1840 hours, Orem City Fire and EMS units were dispatched to a major motor vehicle accident with extrication and ejection reported.
Upon arrival incident command was established by BC McRae. Two medivac helicopters were requested initially along with: Engine 2, Rescue 2, Extrication Unit, Engine 3, Rescue 3 and Rescue 1.
In all there was:
- 1-Fatality
- 1-Critical via air transport
- 1-with minor injuries
- 1-admited to the ICU for observation.
E2, R2, Extrication, E3, R3, R1, BC 1, Air Med responded with additional helicopter requested then cancelled due to 10-85E
