Citations Issued in NY LODD Investigation

The report says Lt. Jack Rose used SCBA from another department and had not been trained on its use.

Two fire departments were issued violations following an investigation into the death of Lt. Jack Rose.

Rose, 19, a volunteer with the Mount Marion Fire Department, responded to a mutual aid house fire in Saugerties on Dec. 19, 2015.

The New York State Public Employee Safety and Health Bureau issued six violations to the Centerville Fire Department and eight violations to the Mount Marion Fire Department, according to the Times Herald-Record

It was determined that Rose took two SCBAs from a Centerville fire truck and officials are not sure why he didn't use the ones on his apparatus. The report says that he had not been trained how to use the other department's SCBA.

“Lt. Rose may have removed his mask in order to obtain these burns,” the report states, noting he suffered second degree burns on his forehead and nose.

The report also said stated that interior and exterior crews failed to keep in contact.

“Additionally, due to the presence of thick smoke and high-temperature conditions, it is believed that Lt. Rose became disoriented as to his position relative to the exit door and thus began searching for an exit.” He was separated from his crew.

The report was conducted by Saugerties police and PESH investigators.
