Carthage Fire Chief Ed Stallings, 71, died over the weekend at a Nashville Rehabilitation Center.
Officials said he never fully recovered after the roof of a burning church collapsed on top of him.
In April Stallings was one of three Carthage fire fighters injured while battling the church fire.
Early Sunday morning, Smith County emergency crews were told Stallings had died.
