"This is not a false alarm."

After hearing that from a 9-1-1 caller and chaos in the background, a dispatcher upgraded the call to Koch Foods plant to a structure assignment.

Multiple workers called to report an explosion and fire. One didn't make it out, WLWT reported, adding dispatchers urged evacuation.

Firefighters would later find Griffin Darrow, 25, dead inside an area that had partially collapsed. Two other workers suffered burns.

OSHA is now one of several agencies involved in the investigation.
