Peer Review Finds Mishandling in TX Blaze that Killed FF

The San Antonio Fire Department mishandled the May 2017 fire that killed firefighter Scott Deem.

According to a peer review conducted by the Houston Fire Department, the San Antonio Fire Department mishandled certain elements of the May 2017 blaze that killed firefighter Scott Deem and badly injured firefighter Brad Phipps.

One of the findings was that the response was treated like a residential fire and not a commercial fire, and another cited "freelancing" on the fireground.
